CHILD PARTICIPATION CONSENT FORM FOR OUTDOOR ACTIVITIESOrganizer: Forest Elf ClubThis form is intended for reviewing and confirming your child’s participation in an outdoor activity. Please read the entire text carefully. By allowing your child to participate in our event, you acknowledge and agree to all of the terms and conditions described below.Terms of ParticipationI, as the parent / legal guardian, confirm that I allow my child to participate in an activity organized by the Forest Elf Club (hereinafter referred to as the Organizer), conducted outdoors.The program may include:- physical activity
- themed games
- role-playing elements
- nature exploration
- simple creative or craft activities
- supervised campfire use for outdoor learning and simple food preparation.
Responsibility for the Child’s HealthI understand that participation in the activity involves physical activity and natural outdoor conditions that may include uneven terrain, weather changes, open fire used for educational purposes, and other elements typical of outdoor environments.I accept responsibility for any injuries that may occur, including but not limited to those caused by:- failure to follow safety instructions by the child
- careless behavior
- actions of third parties
- natural outdoor conditions or other external factors.
In such cases, I waive any claims against the Organizer of the activity, except in cases of proven gross negligence.Compliance with Rules and Safety RequirementsI agree to ensure that my child:- follows all instructions given by the Organizer
- respects the guidelines for participation
- behaves respectfully toward other participants and nature
- does not disrupt the activity.
The Organizer reserve the right to remove a participant from the activity if these conditions are not respected.Responsibility for Personal BelongingsI acknowledge that the Organizer is not responsible for my child’s personal belongings (clothing, shoes, backpack, water bottle, etc.) brought to or left at the activity location.Outdoor activities may involve mud, water, smoke from the campfire, and other natural elements that may result in dirty or wet clothing.
I waive any claims in the event of loss or damage.Medical InformationI agree to inform the Organizer in advance of any relevant health information regarding my child, including:- allergies
- food allergies or dietary restrictions
- chronic conditions
- sensitivity to weather
- other important medical details.
Outdoor Food PreparationSome activities may include occasional outdoor food preparation as part of the educational experience, such as roasting bread on a stick, warming simple food over a campfire, or sharing a warm drink during cold weather.
I understand and agree that:
- food preparation is an optional and occasional activity
- it is not a catering or food service
- children are encouraged to bring their own snack or meal
Food that may occasionally be offered by the Organizer (such as simple soup, pasta, bread on a stick, or roasted vegetables) is provided as a voluntary addition and is not guaranteed as a complete meal.
Children are free to choose whether to taste or eat any food offered.
The Organizer cannot be held responsible for individual digestive reactions or discomfort.
The Organizer cannot guarantee an allergen-free environment.
Participation in food-related activities is voluntary.
Photography and Video RecordingI consent to photos and videos featuring my child being used for educational and informational purposes (for example on the Club’s website or social media), without mentioning the child’s full name, unless prior consent is given
.AcknowledgmentBy registering my child for the activity, I confirm that I have read, understood, and agree to the terms described in this document.
The Organizer reserves the right to update these terms when necessary.
