CHILD PARTICIPATION CONSENT FORM FOR OUTDOOR ACTIVITIES

This form is intended for reviewing and confirming your child’s participation in an outdoor activity. Please read the entire text carefully. By allowing your child to participate in our event, you acknowledge and agree to all of the terms and conditions described below.


Terms of Participation
I, as the parent / legal guardian, confirm that I allow my child to participate in an activity organized by the Forest Elf Club (hereinafter referred to as the Organizer), conducted outdoors. The program includes physical activity, themed games, and role-playing elements.

Responsibility for the Child’s Health
  • I understand that participation in the activity involves physical activity and potential risk of injury.
  • I accept responsibility for any injuries that may occur, including but not limited to those caused by:
- failure to follow safety instructions by the child,
- careless behavior,
- actions of third parties or other external factors.
  • In such cases, I waive any claims against the Organizers of the activity.

Compliance with Rules and Safety Requirements
  • I agree to ensure that my child:
- follows all instructions given by the Organizers,
- respects the guidelines for participation,
- does not disrupt other participants.
  • The Organizers reserve the right to remove a participant who does not comply with these conditions.

Responsibility for Personal Belongings
  • I acknowledge that the Organizers are not responsible for my child’s personal belongings (clothing, shoes, backpack, water bottle, etc.) left at the activity location.
  • I waive any claims in the event of loss or damage.

Medical Information
  • I agree to inform the Organizers in advance of any relevant health information regarding my child, including allergies, chronic conditions, sensitivity to weather, and other important details.

Photography and Video Recording
  • I consent to photos and videos featuring my child being used for educational and informational purposes (e.g., on the Club’s website or social media), without mentioning the child’s last name.
